The etiologies of isolated fourth cranial nerve palsy: a 10-year review of 158 cases

PurposeTo identify the etiologies of isolated fourth cranial nerve palsy in Ramathibodi hospital, Thailand.MethodsPatients diagnosed with from January 1, 2009, through July 31, 2020 Hospital, were included this retrospective, observational case series. The demographic data patients, age at presentation, and neuroimaging results (if indicated) recorded.ResultsWe identified 154 unilateral 4 bilateral cases palsy. Mean presentation was 38.89 ± 25.71 years old. Most congenital (57.79%), microvasculopathy (27.92%), intracranial neoplasm (8.44%) other etiologies. Trauma closed head injury most common etiology (75%), followed by ruptured arteriovenous malformation (25%). Twenty-one 43 (48.84%) patients underwent neuroimaging, normal findings, all patients’ symptoms resolved within 6 months symptom onset.ConclusionsIn our series, congenital, frequency tumor, as many studies. In microvasculopathy, clinical signs cases: observation sufficient, no necessity for neuroimaging. However, should be considered atypical presentations, such headache, periorbital pain, or if there is rapid progression recovery.
